Analysis of Factors Influencing Digital Transformation Readiness in Hospitals Using the Organizational Digital Transformation Readiness and TOE Framework

Abstract

Digital transformation readiness in healthcare extends beyond the availability of technology to the organizational and environmental conditions that shape the implementation of digital solutions in practice. This study aimed to identify and analyze digital transformation readiness components, refine them based on contextual empirical evidence, organize them through the integration of Organizational Digital Transformation Readiness (ODTR) and Technology–Organization–Environment (TOE), and assess stakeholder consensus using the Fuzzy Delphi Method (FDM). A qualitative-dominant sequential design was employed in a single specialized eye hospital involving nine purposively selected participants with domain relevant expertise. Interview data were analyzed using a framework-guided hybrid deductive inductive thematic analysis. The same nine participants subsequently completed the FDM phase; accordingly, FDM was used to assess within-panel stakeholder consensus and refine the candidate components rather than as independent external validation. The analysis produced 23 readiness components, all of which met the predefined FDM consensus criteria and were organized across the Technology (5), Organization (14), and Environment (4) domains. Three context-derived refinements, EM01 Specialty-Specific Features, EM02 Service Continuity, and EM03 Vendor Dependency, were incorporated into the corresponding TOE domains without forming an additional domain. In the context of the specialized eye hospital studied, the findings indicate that digital transformation readiness involves the interrelationship between technological resources and processes, organizational capabilities and governance, human and cultural readiness, and the external digital health environment. The resulting contextual framework provides a traceable structure for understanding and reviewing digital transformation readiness within the organizational setting examined and may support context-sensitive planning for hospital digital transformation.
